This section of eMedNY's website is where facilities record with the New York State Medicaid Program the National Provider Identifications (NPIs) and licenses of practitioners who are affiliated with the facility and who therefore would be included as furnishing, ordering, referring, prescribing, or attending providers on claims submitted by the facility.

It is the responsibility of the facility to ensure that affiliated practitioners are appropriately credentialed, i.e.:

• The practitioner is currently licensed or registered as applicable to their profession.
• The practitioner's NPI is in an active status; and
• For those practitioners who are in a provider category which is enrollable in the New York State Medicaid Program, that the affiliated practitioner has an active provider enrollment status.

Furthermore, completion of this transaction attests that the facility has taken appropriate screening measures to ensure that the affiliated practitioner, if an enrollable provider category, is actively enrolled in the New York State Medicaid program and is not currently excluded from participation in a federal or state health care program (e.g., Medicare, Medicaid).
